Qualcomm Life, DaVita, and P2Link team up to provide connected care for heart-failure patients
Connected health care—and patient-centered care—just took a big leap forward. At its recent Connect 2015 conference, Qualcomm Life (a subsidiary of Qualcomm Incorporated) announced a new collaboration with DaVita Healthcare Partners, a medical group and physician network operator that provides integrated care management for close to 830,000 people.
The aim of the collaboration is to develop chronic-care management solutions for patients with heart failure (HF) that’ll be implemented using Qualcomm Life’s 2net Device Connectivity Platform and the HealthyCircles Care Coordination Platform. They’ll receive a home blood-pressure monitor, a weight scale, and a tablet, powered by 2net Mobile, that will collect and transmit their biometric data to the cloud.
Meanwhile, the HealthyCircles Platform will monitor the patient’s health status at home, and send relevant educational material and proactive care-management interventions to the participants in this program, which is expected to roll out in the fourth quarter of this year.
The benefits for patients of this kind of collaboration are clear: With home monitoring, they can skip traveling to care providers for unneeded checkups, and continuous monitoring can provide a lot of peace of mind. Meanwhile, doctors receive a steady stream of relevant, contextual health information about their patients and can coordinate with other health care providers, and be alerted quickly when problems arise. Cost, complexity, and wasted time are reduced across the board.
P2Link, a provider of health-monitoring solutions, is also collaborating with Qualcomm Life in the U.S. and Europe to deploy remote-monitoring medical device kits powered by the 2net Hub and Platform for patients suffering from HF and Chronic Obstructive Pulmonary Disease.
